A 48 years female patient presented with these asymptomatic lesions since 4 years.

The DD includes the three main granulomatous disorders; sarcoidosis, leprosy and lupus vulgaris.

diagnosis

Sarcoidosis :The patient presented by reddish granulomatous plaques and nodules of long duration, and not involving other sites of the body. No surface changes (no necrosis commonly present in LV or sensation changes of leprosy). The histopathological examination revealed non-caseating epithelioid cell granuloma, in addition to the presence of hypercalcemia and elevated angiotensin converting enzyme confirm the diagnosis of cutaneous sarcoidosis without systemic involvement, particularly chest (free x-ray).

treatment

Treatment is systemic steroids 30-40 mg.
